Introduction
An abortion is the medical process of ending a pregnancy so it does not result in the birth of a baby.
It is also sometimes known as a 'termination' or a 'termination of pregnancy'.
Depending on how many weeks you have been pregnant, the pregnancy is ended either by taking medication or by having a surgical procedure.

An abortion is not the same as a miscarriage, which is where the pregnancy is lost or ends naturally. The loss starts without medical intervention, although medical or surgical treatment may be needed after a miscarriage has started to help empty the womb.

When an abortion can be carried out

Unplanned pregnancies are very common

It’s estimated there are about 300,000 unplanned pregnancies in South Africa every year. There are many reasons unplanned pregnancies happen, including:

-lack of control in a relationship when it comes to sex or family planning
-lack of understanding on using contraception correctly
-a sexual assault.
It is important to remember that many women face an unplanned pregnancy at some stage in their lives.

Medication (non-surgical) abortion offers an alternative to surgery for women in the early weeks of pregnancy. The most well-known method of medication abortion uses mifepristone in combination with another drug, misoprostol, to end a pregnancy 0 to 24 weeks . Mifepristone is also known as RU486 or the ‘abortion pill’.

Medication abortion is a low-risk, non-invasive way to terminate (end) a pregnancy. The success rate is up to 98 per cent, which is only slightly lower than the rate for surgical abortion. In some countries, mifepristone is also used to treat a range of medical conditions, including endometriosis and cancer of the uterus (womb).

Preparing for a medication abortion

Medication abortion involves using medication instead of surgery to end a pregnancy. It can be done as soon as pregnancy is detected and the pregnancy . In South Africa, a medication abortion can be performed up to nine weeks of pregnancy althought some women prefer doing it when it has excessed such a period of time.

Before you can have a medication abortion, you will need to:
to check the stage of your pregnancy
talk with a nurse, doctor or counsellor
see a doctor, who will also ask you about your medical history.
The most well-known and effective form of medication abortion uses mifepristone.

Medication abortion procedure

A pregnancy needs high levels of the hormone progesterone to continue. Mifepristone works by blocking the action of progesterone to cause a miscarriage early in the pregnancy.

If there are no medical reasons that would make a surgical abortion a safer option and you decide to have the procedure, you will be asked to sign a consent form. You will then:
take a tablet of mifepristone at the clinic and then go home
twenty-four to 48 hours later, take the second dose of tablets (this dose contains the drug misoprostol, which softens the cervix and helps the uterus push out the pregnancy)
experience a miscarriage of your pregnancy between one and 24 hours later.
You will then:
Come back to the clinic in two weeks for a follow-up appointment to make sure the pregnancy has ended (this may involve having blood tests or an ultrasound)

What to expect after a medication abortion

After you have taken mifepristone, you may have experience nausea for a short time and light bleeding.

After you have taken misoprostol, you may experience nausea, vomiting headache, diarrhoea and chills. You may also have heavy bleeding and cramps that may be heavier and more painful than a normal period, usually beginning within four hours. The most severe symptoms usually settle within a few hours once the abortion is complete.

Follow-up after a medication abortion

You will need to have a follow-up appointment with our doctor or healthcare professional to confirm the pregnancy has ended. At this appointment, you may have an ultrasound (and possibly a blood test) to make sure the termination is complete. In about two per cent of cases, the abortion is not complete and a surgical termination is needed.

Self-care at home

You should take care of yourself in the 48 hours after having the procedure by following the advice of your doctor or healthcare professional. This can include:
using over-the-counter anti-inflammatory medication to help manage the cramps (your doctor can prescribe stronger medication if needed)
not smoking, drinking alcohol or using illicit drugs.
To lessen the risk of infection during the week after having the procedure, you should not:

HERE ARE THE INSTRUCTIONS FOR THE ABORTION PILL
1. Day 1, Mifeprex: When you are still in our office you will take the Mifeprex
(mifepristone) pill with water or other liquid. This is considered Day 1. Eat and drink
normally so you do not get dehydrated. You may have some symptoms like headache,
nausea, dizziness, or more likely, some cramps or a little bleeding. Most women do not
have these symptoms, but if you have cramps, we recommend Tylenol Extra Strength,
Ibuprofen, or if the cramps are bad, take one of the pain pills we gave you. If you have
hives (skin rash) or other reaction to the medicine call our office at (012) 752-7398. Start
antibiotic TONIGHT as directed.
2. Day 2, Misoprostol: Within the next 24 hours, at your convenience, take 4 tablets of
misoprostol either by letting it dissolve in your mouth between cheek and gum, or by
swallowing. There may be fewer side effects like nausea or diarrhea letting it dissolve in
your cheek. Gum or candy may help with the chalky or bitter taste. You can pick
whatever time is convenient for you 12 hours after the Mifeprex. If you have childcare or
other responsibilities, please make arrangements for help in case you need it.
3. Cramping can start in an hour or longer. You may want to take some pain medication
at the same time you take the misoprostol. Many women cope well with Ibuprofen 800 mg
(4 tabs of over the counter Motrin, Advil type product) or 2 Aleve tablets (Naproxen
Sodium). Or, you may take one tablet of the pain reliever we gave you. If cramping is
severe, you can alternate Ibuprofen two hours after the Vicodin.
4. Bleeding can start as soon as the first hour, but may not begin for as long as 4-8
hours. After the bleeding starts it may be heavy for 3-4 hours. Most women will pass the
pregnancy during this time, although sometimes it will take longer. Clots can get very big,
as large as a golf ball, or even a tennis ball size. Bleeding may also be heavy: using the
full-size maxi-pads please call us if you saturate two pads an hour for two hours.
Saturating means that if you wrung out or squeezed the pad, blood would drip. It may be
kind of rough for a while, but after you pass some clots it should gradually get better.
5. Fever: Don’t be alarmed if you have a low grade fever, but if you feel chilled, dizzy,
nauseous, or shaky for more than 4 hours, with or without a fever or elevated
temperature, we want to hear from you. We want you to take an antibiotic in three
doses starting the evening of Day 1. If a fever continues or is over 100.4 for more than
four hours, call us. These may be side effects of the misoprostol, but they may also be
signs of an infection which is rare, but potentially serious.
6. Passing the pregnancy: Most women will pass pregnancy tissue (the gestational sac,
which contains the embryo) sometime in the first 4- 8 hours. Some may take longer. It
may be inside a clot or be more recognizable as a white or tan translucent sac. If you are
not certain that you have passed it, you can stop by for a check-up this week. Call for
an appointment. If you had pregnancy symptoms, like nausea, you should start to feel
better in the day after you pass the pregnancy.
7. What if it doesn’t work? If you did not bleed much at all, or if your pregnancy
symptoms persist for more than two days, you may not have passed the pregnancy. If you
are unsure, come back for a check-up. If you have not passed the pregnancy, you can
either take another dose of misoprostol at no extra charge in our office, probably on a Tuesday, Wednesday, or Thursday, when Dr. Johnny is available. Please call first to discuss this.
8. Too much bleeding: You can have a lot of bleeding with this method, particularly if
you are closer to 7 weeks LMP. If you are bleeding more than is described in #4, please
call us. If you are bleeding heavily for more than 2 weeks, please call us. Follow the
instructions carefully and call if your bleeding is more than we told you to expect. After we
evaluate your condition we will tell you if we feel you need some medicine or to go to the
Emergency Room. It is rare to have an emergency, but we have someone on call if you are
experiencing a serious problem. Have the number of a pharmacy that is open when you
call.

Pregnancy hormone can be in your body up to 4 weeks after a medical abortion. Please wait till you have a follow up visit to the clinic (in 2-3 weeks) and we will perform all the necessary tests to make sure you are no longer pregnant.

How long will I bleed after Medical abortion?

In most cases the patient experiences bleeding for about a week after the abortion. However, some patients do experience bleeding for 3 days and other patients have spotting or bleeding off and on. It is important that you return for your follow- up evaluation one week after your procedure.

When can I have sex after the Medical abortion?

You should not have sex for two weeks after your procedure. When you return for your one week check up, the doctor will advise when you can resume sexual intercourse. This is because you are susceptible to infection.

Is Abortion painful?

How will I feel after my Medical abortion?

What must I do before the surgery?

You must not have any thing to eat or drink for a least 4 hours before the procedure. All nail polish must be removed and please do not wear any jewellery.

Why can't I eat before Medical abortion?

Because when anesthesia is administered, gag reflexes are temporarily suppressed. If food or liquid remain in the stomach, vomiting can occur resulting in choking.

Is it a doctor who performs the abortion?

Yes, the doctor is a specialist Ob-Gyn who is specifically approved by the MOH, South Africa to perform abortions.

Is dangerous to have an abortion?

There is no minimum age and parent's consent is not legally required. However girls under 16 years of age need to have counseling at the Institute of Health before having the pregnancy terminated. If I get a negative result on a home pregnancy test, does that mean I am not pregnant?
A negative result can mean that you are not pregnant, you took the test too early, or you took the test wrong. Pregnancy tests vary in their sensitivity (how soon they can detect the hormone hCG), and you may not have given your body enough time to produce enough hCG hormones that will show up on the test. Also, if you let a test sit for too long (after the instructions on the box tell you), the test is invalid. It is best to follow the instructions and wait until you have missed a period before taking the test. Most pregnancy tests come with two in a box, and it is a good idea to take both.

When is the earliest that I can take a home pregnancy test?
It is recommended that you wait until you have missed a period to take a home test. A missed period is often one of the first signs of pregnancy. What if I take a couple of home pregnancy tests and get different answers?

Making a decision

When you have made your decision, stay strong. If you start letting people make you doubt your decision, it can confuse you all over again. If people don’t respect your decision, they’re not worthy of your friendship.

ZINTLE
Because all my friends knew I was pregnant, it worried me stupid that if I didn't have the baby everyone would know I’d had an abortion. Having a baby doesn't have the same judgment and stigma attached to it. I wanted to know if my friends were going to be there for me or not. I rathered that people knew so that I knew what support I had. Even if I’d made the decision not to have the baby, then I might have someone say down the track, ‘It would have been great’, and that they’d have given me support, and I wouldn't have known that.
I would have loved to say, ‘I’m pregnant’, and be really happy about it. That’s what I really wanted. It’s an added stress when you’ve got everyone’s opinions flying at you. If you’re going to tell anyone make sure your secret is safe.

Abortion

KATLEGO
I had three children already and the fourth pregnancy was an accident. My relationship with my husband wasn't very stable and he felt another baby would jeopardize the family we already had and further destabilize our relationship. Although part of me would've been ok about having another child, I was scared about coping and perhaps not having the support from my husband, and even the possibility of my marriage ending if I had continued with the pregnancy. Although not without its emotional difficulties, I don’t regret having had an abortion. It was the right decision for my family at the time and now I can look back and see it was the right decision for me too.

SHARON
It just wasn't right for me and my partner at the time… Having a kid would have been awesome but it just would have been too hard… Like if we had have been together longer and like we weren't in uni and things, it might have been different, but just debt, debt. It wasn't really an option…

Myth 1: Abortion leads to infertility, ectopic pregnancy and breast cancer
Fact 1: Research has shown no link between abortion and infertility, ectopic pregnancy or breast cancer

The Better Health Channel, notes that while all surgery carries some risk, abortion doesn’t automatically harm fertility. A safely performed abortion is ‘10 times safer’ than pregnancy that continues to term in developed countries. Studies have shown no relationship between abortion and breast cancer.

Fact 2: The majority of women cope well after an abortion

The Pregnancy Advisory Service at Connect Women's Center has been supporting women through abortion for around 10 years. A majority of these women report that they did not find abortion emotionally or psychologically harmful. Although some women find it stressful or difficult in the short-term, most women cope positively and do not experience long-term problems.

It is important you find someone supportive and unbiased who you can talk with about your decision. This could be a friend, family member, partner, counsellor, GP or a community member. Try to talk to someone who understands abortion is an option – someone who acknowledges that you have right to decide what’s best for you and your pregnancy.

Myth 3: Women don’t have the right to decide what’s best for them
Fact 3: Women know best about what to do about their unplanned pregnancy

Supportive people can certainly help you think through your options. Women tell us their life decisions are connected with those around them. However, someone else should not make the decision for you. You know best about your own life circumstances, your strengths and capabilities, as well as your resources. You are also the main person who will be affected by continuing with the pregnancy or having an abortion.

Recent studies have shown that coercion around reproductive choice is often connected to partner violence or abuse. In other words, do you have a partner who is trying to force you into having a baby? Or having an abortion?

Myth 4: You are always in control of whether or not you get pregnant

Fact 4: Fertility is not always within the control of women

The fact is that:

contraception is not always 100% available and foolproof
women are not always in a position to consent to sex.
Studies have shown that between half and two-thirds of all women who have an abortion were using contraception at the time they became pregnant. No contraception is guaranteed to be 100% effective all the time. Sex and contraception are two areas where human beings and their relationships can be unpredictable and disorganised. The circumstances of women’s lives and their health – irregular periods, menopause, drug use, medical conditions, stress – will also affect fertility and contraceptive use.

Furthermore, sex is not always voluntary. Many women seeking abortion have not been in a position where they could freely agree to sex. This may apply to women who are in or outside of a relationship.

Myth 5: Women who have abortions don’t value motherhood
Fact 5: Abortion and motherhood are not in opposition to each other

Many of the women we talk to are already mothers. Often their decision is influenced by the desire to do the best they can for the child or children they already have. They fully understand the responsibility of parenting and what it would mean to have another child without having adequate resources to ‘do it properly’. This puts them in the best position to make a decision about whether to continue with a pregnancy.

There is also a myth often used to create a false division between ‘normal’ women, who are ‘natural’ mothers and ‘abnormal’ women, who do not become parents for a variety of personal reasons. This division is insulting – and not only to women who choose not to parent. Men’s choices about whether or not to have a family, for example, are rarely judged in the same way. Many women who want to be or are already mothers may also experience an unplanned pregnancy and an abortion in their life. Indeed, PAS statistics from 2005 show that at least half the women contacting the service with an unplanned pregnancy already had children.
